Includes administrative tasks like reporting, contact tracing, and referral.</span></li><li><span>Provides education, support, and referral to individuals with chronic disease.&nbsp;</span></li><li><span>Health Education for general community members based on assessed needs and health trends.</span></li><li><span>Through health centre visits, clinics, or home visits, deliver direct nursing care per BCCNP standards and Lil’wat Health and Healing guidelines and provide care management to assist clients to manage their own care and navigate through various health services.&nbsp;</span></li><li><span>Promote programs and educate the community about internal and external programs and services and work to ensure access for eligible community members.</span></li><li><span>Creates supports and delivers culturally relevant activities to educate and inform community members on communicable diseases, vaccines, health promotion, and injury and illness prevention.</span></li><li><span>In consultation with Maternal Child Health Coordinator and the team performs duties and supports programming including prenatal and postnatal health, newborn home visiting and assessment, breastfeeding support and ongoing monitoring and support for children birth to 6 years and their families.</span></li><li><span>Maintain collaborative partnership with pharmacist to provide support with Indigenous health, community-based projects and engagement, and initiatives related to cultural safety and decolonization of pharmacy practice.</span></li><li><span>Ensure client documents are maintained and updated to include all required information and ongoing client charting.&nbsp;</span></li><li><span>Completes all required reports and maintains a database of community health information.</span></li><li><span>Perform other related duties as requested.</span></li></ul><div><span><b>Experience and Attributes:&nbsp;</b></span></div><div><span><br></span></div><ul><li><span>Current practicing registration, in good standing, with the British Columbia College of Nurses and Midwives (BCCNM).</span></li><li><span>Degree in Nursing from a recognized university.</span></li><li><span>Current BCCDC Immunization certification.
